Who We Are
Founded in 1994, Health Choice Network of Florida (HCNFL) is a model of successful collaboration among
community health centers. As a Health Center Controlled Network, it is a not-for-profit organization
governed by a Board comprised of its members. Network members serve nearly half a million patients,
many without health insurance. By providing the tools to strengthen our members and partners, Health
Choice Network Florida improves the lives of the underserved and vulnerable in our communities.

The South Florida Regional Extension
Center (SFREC) is one of 60 centers
nationwide helping local physicians and
health care providers select, successfully
implement and meaningfully use certified
electronic health records (EHR) to improve
the efficiency, quality and value of health
care.
The Medicare and Medicaid EHR
Incentive Programs will provide
incentive payments to eligible
professionals, eligible hospitals and
critical access hospitals (CAHs) as
they adopt, implement, upgrade or
demonstrate meaningful use of
certified EHR technology.
Prestige Health Choice, a capitated
provider service network owned by
HCNFL and its centers, continues to
focus on high quality patient care
while adding members throughout
Florida. Prestige is a Medicaid
health care plan with more than
60,000 members in 24 Florida
counties
